Please direct me to a website or better still an old template i can use a guide.
Thank you.

Theres a couple of threads based on this.
I had one a couple of years ago ill find it and post it here.
For my home environment am using OneNote to document everything, from the servers, switches, ip addresses.
Attached on the second page as

The question that I would ask first, is what do you think you will use that documentation for (or what will it be used for; not quite the same thing). That will determine what you need to do.
You can generate a simple map with some indications of basic IP addressing; often all that most people want. However, you can also produce very detailed documentation that shows IP addresses, MAC address, port allocation information, VLANs etc.
Personally, I use Visio; it allows the mapping, and I can use it to show how things are structured within a rack, how VMs work within a virtual landscape. (e.g. links from vNics to vSwitch to pNics). I can also generate a single page for each switch or WAP, with appropriate information on configuration (and where the backup config files are located)
