I’m looking for an outlook form or template for different departments to have a checklist of items to be completed for a new hire.
I would like a outlook form, but any template would be great.

Do you have Sharepoint? I created a custom list with all of those things, including checklists for the Admin’s to make sure it got done. I then logged into Sharepoint using the HelpDesk user name that I created for Spiceworks and turned on notifications so anytime something new goes into the custom link Sharepoint sends a notification to my Spiceworks email address and I get an automatic ticket.

I was going to recommend Sharepoint also. That way you don’t have to deal with “I never got the email”.

I always forget that part of doing a cool project.
I use a service at www.jotform.com
It’s free (up to 10 submissions per month) … we’ve converted many paper forms using this… One of the first forms I completed was a new hire form for managers/HR to complete which automatically submits the completed form to the HR manager, as well as the helpdesk, which automatically creates a ticket… it’s pretty slick IMO. Then you can embed it in the portal to provide access.
Submissions can be PDF’s or just html… I’d provide a screenshot or link to my form for reference… but it’s 2 pages so it’s hard to get a screenshot and the link would allow you to fill it out and submit it.
